Medication Errors

A medication error is when a preventable event that can cause or result in the inappropriate use of medication use or patient harm while the medication is in the control of a health professional or patient. The events can be related to procedures or practice, such as: prescribing, dispensing and administration, and generally become actionable when the mistake results in a personal injury.

A study recently conducted by Harvard School of Public Health concluded that more than 43 million people worldwide are harmed each year by unsafe medical care. One of the most common reasons for the associated personal injury was found to be medication errors. A study done by the Institute of Medicine (IOM) of National Academies also found that more than 7,000 deaths a year are attributed to medication errors and negligence. Boston Personal Injury Lawyers, including those at Mass Injury Firm, P.C., have paid personal attention to the studies. The most common medication error, we find, is mistake caused by medical abbreviations resulting. This type of mistake is particularly prone to cause personal injury becuase it is completely arbitrary.

Certain abbreviations, symbols and dose designations have been found to be frequently misinterpreted leading to medication error that results in personal patient injury. The errors occur in medical communication, such as: prescriptions, medication labels, administration records, computer order entry screens and commercial packaging. The U.S. Food and Drug Administration (FDA) and Institute for Safe Medication Practices (ISMP) launched a national campaign in 2006 aimed at reducing medication error resulting from confusing and mistake-prone medication abbreviations.
